Private Membership Club Agreement

I, , for membership fees due, do hereby apply for membership into “Club Bliss”, a private membership club. With the signing of this membership agreement, I accepx the offer made to become a member. I have read and agree with the following Declaration of Purpose.

DECLARATION OF PURPOSE

1. This club of members hereby declare that our primary purpose is to protect and maintain our right to freedom of choice regarding alternative therapies, alternative modalities of treatment, wellness care decisions and the health improvement practices that we choose to receive - by asserting our constitutional, contractual, and civil rights.

2. As members, we affirm our belief that the Constitution of the United States guarantees all Americans, particularly members of private associations or clubs, the right of freedom of association, speech, assembly, belief, and associated activities. These are our inalienable rights.

3. We declare and assert the right to select from our membership those who can be expected to give the wisest counsel and advice regarding alternative therapies, alternative modalities of treatment, wellness care decisions and the wellness improvement practices and to authorize those members who are most skilled to facilitate the actual performance and delivery of any wellness assistance and or improvement methods that they and we deem appropriate. We assert these rights under the Federal and State Constitutions, Federal and State law and the statutes and regulations interpreting them.

4. We claim our freedom to choose and accepx for ourselves the types of wellness care modalities that we think are best for determining the cause and correction of our life challenges. We do this in order that we might achieve opximal well-being and overall happiness. We reserve the right to include traditional, non-traditional or even unconventional wellness opxions, plus other healing modalities or techniques that our members or facilitators choose to deliver, through uBlissity Inc. executive management approval.

5. More specifically, our mission is to provide members with the highest quality wellness services available. Our concern is for the whole person - body, mind, and spirit. We strive to stay on the leading edge of new and improved wellness technologies.

6. Club Bliss recognizes all persons as members, without respect to race or religion, who are in alignment with our principles and policies, however some services have user limitations and use of some services are dependent on the members’ personal circumstances.

I understand and accepx that Club Bliss is protected by the First, and Fourteenth Amendments to the United States Constitution, it is exempx from any action of Federal and State agencies entrusted to “Protect the Public” – as it relates to any complaints or grievances against Club Bliss, uBlissity Inc., its physical premises or equipment, its Trustees, members - facilitators or other associated staff or consultants.

THIS MEANS THAT OUR CLUB ACTIVITIES, SERVICES AND MEMBER INTERACTIONS ARE RESTRICTED TO THE PRIVATE DOMAIN.

All complaints or grievances will be settled by non-judicial mediation, within the Club. Also, those membership and private member records kepx by the Club are strictly protected and can only be released upon written request of the subject member.

As a Member :

• You will only choose to accepx the wellness services that you feel will truly help you.

• You will only choose procedures that you consider proper and that you feel have a reasonable chance of making your life better.

• You acknowledge that no screening, resulting conclusions or wellness care services are foolproof.

• You acknowledge that if you choose to forgo drugs, surgery or sympxom based treatments that have been recommended by others, like your primary care physician, YOU fully accepx that risk.

MEMORANDUM OF UNDERSTANDING (Please initial below)

I understand that employees of uBlissity Inc. and fellow Club Bliss members provide all services and do so in the capacity of a Private Member and not in the capacity of a retailer, employee, owner or licensed provider. I understand that uBlissity Inc. only provides Club Bliss members with the appropriate space, equipment and time, assisting the private club member with their desired ambiance.

In addition, I have freely chosen to change my legal status from a “public consumer / customer” to a “private member” of Club Bliss. I further understand that it is entirely my own responsibility to consider all discussions, recommendations and products offered to me by my fellow members and to educate myself as to their efficacy, risks. The desirability of the same and the accepxance of the offered or recommended products is my own carefully considered decision. Any request by me to a fellow private member to assist me or provide me with the aforementioned recommendations or products is my own decision in the exercise of my rights and made by me for my benefit, and I agree to hold Club Bliss, uBlissity Inc., its staff and other workers or any other Club Bliss members harmless from any and all unintentional liability for the results of any such services or recommendations and / or products, excepx for harm that results from instances of a clear and present danger of substantive evil as stated and defined by the United States Supreme Court.

uBlissity Inc. has been chosen as the entity best qualified to provide services to fellow members of the club and entrust them to select other members, when needed, to assist them in carrying out the various Club Bliss services.

I understand that Club Bliss is protected by the first and fourteenth amendments to the United States constitution, all Club Bliss interactions are outside the jurisdiction and authority of federal and state agencies and authorities concerning any and all complaints or grievances against Club Bliss, uBlissity Inc.; as a corporation or any club members.

ALL RIGHTS OF COMPLAINTS OR GRIEVANCES will be settled by uBlissity Inc. executive management and SHALL be waived by the member for the benefit of Club Bliss and its members. Because of the privacy and security of membership records maintained within Club Bliss, which have been held to be inviolate by the U.S. Supreme Court, the undersigned member WAIVES the complaint process.

Any customer/consumer records kepx by Club Bliss will be strictly protected and only released upon the written request of the member, and as a member I agree that the violation of, or any waivers in, this membership contract will result in a no contest legal proceeding against me.

I understand that Club Bliss does not participate in any insurance plans.

I agree to join Club Bliss, a Private Membership Club under common law, whose members seek to help other members protect their rights and in their pursuit of happiness.

I understand that the providers, who are fellow Club Bliss members, may offer me products, services and benefits that do not necessarily conform to conventional products or services on the market.

As a private member my goal is to achieve greater happiness by making conscious choices, realizing that no product testing is foolproof. Other aspects of informed consent will take place in my discussions with my fellow private members of the club.

I understand that uBlissity Inc. is a reseller of wellness equipment offered at Club Bliss. If I choose to purchase any of the service products offered at Club Bliss I agree to purchase all of said equipment through uBlissity Inc.

My activities within the Club are a PRIVATE matter and as such I will not share and I will refuse to share any of my activities with the State Medical Board(s), the FDA, FTC and any other governmental agency unless ordered to do so through a court order. All records and documents remain as property of Club Bliss, even if I receive a copy of them. I agree not to file a liability lawsuit against the uBlissity Inc. or any members of the Club Bliss, unless I have been exposed to a clear and present danger of substantive evil.

I acknowledge that Club Bliss members do not carry liability insurance.

uBlissity Inc.’s executive management shall have the right to sanction a member after a hearing of the facts where the member may be present, after notification. The sanctions may include removal from active membership or imposing any other special and necessary conditions upon any member who shall discredit or bring harm to Club Bliss or the Theraphi technology in any manner.

I enter into this agreement of my own free will or on behalf of my dependent without any pressure or promise of products or results. I affirm that I do not represent any State or Federal agency whose purpose is to regulate and approve products.

I have read and understand this document, and my questions have been answered fully to my satisfaction. I understand that I can withdraw from this agreement and terminate my membership in this Private Membership Club at any time with a 30 day written notice.

These pages consist of the entire agreement for my membership into Club Bliss and they supersede any previous agreement(s) either verbal or written.

I understand that the membership fee entitles me to receive those benefits declared by uBlissity Inc. to be “general benefits” free of further charge. I agree to pay as levied those benefits that I receive that are declared by uBlissity Inc. to be “special assessments”, per the fee schedule.

For the sum of $20.00 as consideration for my lifetime membership contract, said term beginning with the date of the signing of this contract, and by these presents do hereby certify, attest and warrant that I have carefully read the foregoing Club Bliss contractual application for membership, and I fully understand and agree with the same.
